Transition in the Prosthetic Service - Toolkit Questionnaire

The Prosthetic Clinical Transitions (PCT) Questionnaire, supports the Prosthetic Clinician Transition Toolki. It is a self-assessment tool designed to help young people with limb difference or amputation prepare for the transition from paediatric to adult prosthetic services. It allows clinicians to identify areas where a young person is confident and areas where they may need additional support.
